ACCEPTED INSURANCE POLICIES
At our practice, we are proud to be in-network with most major insurance providers, including Aetna, Anthem, Cigna, HMA, Kaiser Plans, Lifewise, Meritain Optum, Premera (BCBS), Regence (BCBS), United Healthcare, and UMR. We also handle Labor and Industries (L&I) claims and Personal Injury (PIP claims). Please note that we are not contracted with Apple Care, Molina, or Medicaid plans. For Medicare visits, a $45 fee will apply for any manual muscle work, along with additional fees for necessary exams not covered by Medicare. ESTABLISHED PATIENT FORMS
Here you will find all necessary documents needed for established patients with new complaints or who have not been seen for more than 3 months. Please complete all documents prior to your visit and either email the competed forms or print and bring them with you to your visit.
